Before players may participate in any practice, parents/players must complete all AISD forms that are found in Rank One.

Online forms are now live and ready on RankOne. There are five forms to fill out. “Yes” answers to the medical history page require further explanation before athletes are cleared to participate. All players must have a physical with the exam date within the last year before they are eligible for practice. These are turned into the coaches.

The physical form and medical history page are the only forms to download and turn in. All other forms are signed submitted electronically.

We are excited for the Spring Season with lots of tournaments to be played and lots of growth (Emotionally, Mentally, and Athletically) to occur during the course of the season.
